How to apply for funding:
To apply to the Office of Aboriginal Health for funding you will need to:

Service Report
When you have received funding, you will need to complete a service report.

The Service Report Template has been compiled by the Office of Aboriginal Health to assist organisations to report on the agreed contract measures. Organisations who do not have to use the template will still need to ensure that your organisation's service report provides information on the contract measures.
